Please take a minute to complete the following forms prior to your initial office visit:

Services:

New Patient visit:$325

Typically a 2-3 hour conversation.

Includes:

-comprehensive medical history and physical exam

-review of previous medical records

-thorough review of existing supplement regimen and what to take moving forward

-detailed discussion of how to approach diet, tailored to the individual

-includes ongoing email communication for clarification and support

-will review testing options if proceeding with subscription

For the initial visit, please bring:

1. Any prior lab results or medical records 

2. The BOTTLES of any supplements that are being taken

3. The completed intake forms above (New Patient Info and Symptom Questionnaire)

Insurance Information

In order to ensure that appropriate time can be spent with each patient, the office is not contracted with third party insurance payers.  Fee is expected at the time of service, and a receipt with appropriate diagnosis codes will be furnished for you to submit to your insurance company.
